Your child is required to be fully immunized before attending school in September. They are also required to have a health appraisal done as well. If you haven’t scheduled an appointment please do so as soon as possible.
If your child has a fall
birthday the physical can wait until then but the immunizations must be
complete prior to starting school on September 7, 2016.
ENTERING GRADES 2 & 4:
NYS Education law requires a health appraisal for the
coming school year.
If you haven’t already done
so, please schedule an appointment and send in the health appraisal form to
your school’s health office. If you need a copy of the form use the link below:

before school starts please call the health office with the date of your
appointment.
ENTERING GRADE 6
Your child is required to
have the Tdap and the second Varicella vaccines before the start of school in
September. If you haven’t already done so please make an appointment and send
the paperwork in prior to the start of school on September 7, 2017.
If your child turns 11 yrs.
after the start of school they must receive the vaccines within two weeks of
turning 11 in order to avoid being excluded from school.
